This Targeted Case Manager (TCM) will provide direct supervision and support to the TCM staff with regard to services provided, client interactions, documentation and billing. This position will assist and lead the TCM's through training, daily/weekly supervision, assigning cases from inception through completion.  This position will also facilitate staff meetings and training, and provide oversight for the quality management of the program. 

Responsibilities

  • Recruits, hires, trains and manages Targeted Case Managers for assigned office
  • Certifies that all clients meet criteria to receive TCM services
  • Ensures staff have working knowledge of resources in services area and effectively link clients to appropriate programs, services, and resources in the community
  • Ensures staff complete monthly home visits for assigned cases and provide ongoing assessment of the successes, barriers and compliance with treatment for all providers
  • Ensures staff are participating in court hearings, dependency staffings, education meetings, and psychiatric appointments for advocacy as needed
  • Performs monthly audit reviews to ensure compliance
  • Reviews TCM case documentation to ensure quality and compliance per the Florida Medicaid Mental Health Targeted Case Manager Handbook
  • Reviews charts on a regular basis to ensure documentation and files remain in compliance with the Florida Medicaid Mental Health Targeted Case Manager Handbook and company guidelines
  • Ensures staff complete all Emotional Behavior Assessments and Service Plans within 30 days from date of intake, and completes updates every 180 days
  • Reviews client files at time of discharge to ensure quality and compliance
  • Reviews and certifies TCM billing events for quality and to ensure compliance with the Florida Medicaid Mental Health Targeted Case Manager Handbook and
  • Review and sign all Emotional and Behavioral Assessment/Service plans as well as individual service plans
  • Ensures reimbursement for TCM services are provided by ensuring that all payor procedures are following to obtain authorization for services, monitoring service authorization expiration/utilization on a weekly basis and appealing all billing events that can be recouped.
  • Conducts quality callback surveys as needed to ensure client satisfaction with TCM services, provide copy of survey to Clinical Director
  • Conducts regular supervision (no less than once a month) with each TCM and complete a corresponding supervision note
  • Monitors and manages performance of staff regarding productivity, service and documentation quality, and compliance will all applicable policies, procedures and guidelines; addresses performance issues per company procedures
  • Conducts department meetings and trainings that focus on compliance, quality enhancement, and skill development
  • Complies with, and ensures all staff comply state and federal statues, rules, and policies that affect the target population
  • Meets and maintains qualifications for Behavioral Health Case Manager certification as outlined by the Florida Certification board and provide documentation of such

  • Acts in accordance with professional and ethical guidelines
  • Follows all regulations as outlined in the Florida Medicaid Mental Health Targeted Case Manager Handbook, and ensures all staff adhere to the same regulations
  • Ensures compliance with all regulations pertaining to confidentiality and HIPAA, SequelCare of Florida Policies and Procedures, and Medicaid Assessment requirements
  • May be required periodically to manage a case load, when dictated by census fluctuation or staffing shortage
  • Other duties as assigned

Qualifications

  • Minimum Qualifications: Bachelor's Degree from an accredited college or university in human services, clinical social work, psychology, or related field with three years full time or equivalent professional experience serving the target population; or a Bachelor's Degree from an accredited college or university in an unrelated field with five years of full time or equivalent case management experience servicing the target population
  • Preferred Qualifications: Currently hold a Behavioral Health Case Manager Supervisor certification through the Florida Certification board; two years of management or leadership experience. 
  • Computer Skills: General computer and Microsoft office experience required; Medik and EMR experience preferred
