The Director of Case Management will be responsible for the supervision of the transitional case managers and the case managers to ensure that all transitional planning and day to day services and needs of the students are met.

Responsibilities

  • Completes and assists case managers with documenting student histories as assigned in a timely and thorough fashion as prescribed in program policies.
  • Completes and assists case managers with individual service plans as assigned, meeting content and time criteria as outlined in program policies.
  • Conducts and assists case managers in group meetings and individual sessions, using professionally prescribed methods and techniques to enact positive change for residents.
  • Coordinating execution of Family Day.
  • Completes documentation requirements in accordance with program policy and procedures.
  • Initiates family contact using professionally prescribed methods and techniques to convey/receive information and enact positive change.
  • Interacts with students in a professional manner to elicit positive change.
  • Completes other reports, assessments and reviews, as assigned, in a thorough and timely manner.
  • Coordinates admissions, treatment, and discharge documentation and planning for students with other case managers.
  • Oversees documentation, billing, and compliance with standards.
  • Completes monthly chart audits/peer chart reviews.
  • Instrumental in resolving program grievances and treatment issues.
  • Reviews RIRs daily and provides updates when needed.
  • Facilitates Unit Management Team meetings for monitoring of students and services provided.
  • Provides training and assistance to case managers and staff.
  • Oversees completion of Outcomes data (CAFAS).
  • Adheres to the policy and practice concerning issues of creation, editing, transmission, storage and disposition of all Protected Health Information (PHI) of Sequel Youth & Family Youth Services.
  • Supports by actions and example the Sequel TSI Creed.

Qualifications

  • Education: Master degree in psychology, counseling or related field.
  • Experience: Two years practice in the field working with children with behavioral and emotional problems.  Previous experience in a residential setting preferred and supervisory experience preferred.  Experience with multi-disciplinary team preferred.
  • Computer Skills: Basic computer skills, including usage of email, Internet Explorer, and print documents.
  • Other Requirements: Fluent in the English language to interact with governing bodies, administration and the community both in person and over the phone.
