We are looking for an eager, compassionate case manager to join our team! We are growing and building new partnerships in the local community. We are looking for a case manager who is willing to jump into new situations to assist and support children, adults and families in need in the Greater Augusta area!

Our case managers work in the Behavioral Health Home program and work with children and adults. The right candidate will truly love helping people and will have exceptional time management skills. This position will start part-time, building your client base along the way. There will be other opportunities for you to pick up hours doing direct care service. This would allow you to expand your experience in the field, gain additional training and clinical supervision and be a multi-faceted member of our Behavioral Health team.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
  • Handle case assignments, write treatment plans and progress notes, review case progress and determine case closure
  • Help clients achieve wellness and autonomy
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations with team members throughout the entire case management process
  • Meet with your clients regularly in their home and local communities
  • Link clients and families with local resources, agencies and providers

Qualifications

  • Bachelor's in human services or related field
  • MHRT-C preferred; Willingness to obtain a MHRT-C within the time frame required
  • Minimum of 1 year of case management preferred
